Personalized Home Healthcare Professional

We are seeking a skilled and compassionate home healthcare professional to join our team. As a Home Health Registered Nurse, you will have the opportunity to make a meaningful impact on the lives of patients in their own homes.
Your role will involve providing personalized, one-on-one care that helps patients regain independence and manage chronic conditions. You will work closely with a dedicated team of physicians and clinicians to develop and manage care plans that support recovery and promote whole-person health.
The ideal candidate will possess strong organizational and communication skills, as well as experience working in home health or a related field. A valid driver's license and reliable transportation are also required.
In this role, you will have the opportunity to:
- Provide admission, case management, and follow-up skilled nursing visits for home health patients
- Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
- Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment
- Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems
