This position offers a unique chance for an experienced physical therapist to join a dynamic healthcare team in a Skilled Nursing Facility setting.
We are seeking a skilled professional who can deliver high-quality patient care, assess patients' needs, and develop personalized treatment plans to achieve optimal outcomes.
- Evaluate patients' physical abilities and design evidence-based treatment plans to promote recovery and independence.
- Collaborate with healthcare professionals to ensure seamless patient care, providing education on exercise programs and injury prevention techniques.
Responsibilities:
- Monitor and record patient progress, making adjustments to treatment plans as needed.
- Develop and implement interventions in diverse settings, prioritizing patient safety and satisfaction.
- Contribute to discharge planning, coordinating post-rehabilitation care to ensure continuity of services.
Requirements:
- Doctorate in Physical Therapy from an accredited institution
- Valid licensure as a Physical Therapist in Massachusetts
- Current certification in Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR)
