Home Health Care Specialist
At our organization, we're dedicated to providing exceptional home health care services that cater to the unique needs of our patients. As a Home Health Registered Nurse, you will play a vital role in delivering personalized care and support to those who require it most.
The primary responsibilities of this position include:
- Providing admission, case management, and follow-up skilled nursing visits for home health patients
- Administering on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
- Conferencing with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment
- Providing hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision
- Revising plans in consultation with physicians based on ongoing assessments and as required by policy/regulation
- Coordinating appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers)
- Reporting patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis
- Implementing patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems
- Monitoring the completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility
- Discharging patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation
- Preparing medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes
