We are seeking a detail-oriented and caring Licensed Practical Nurse to join our team of dedicated healthcare professionals.
This challenging yet rewarding role offers the opportunity to work independently and collaboratively with others to deliver high-quality patient care in the comfort of our patients' homes.
SalaryThe salary for this position is $49,100 - $67,500 per year, based on full-time employment (40 hours per week).
Pay may vary based on individual skills, knowledge, experience, education, and certifications.
Job DescriptionIn this role, you will be responsible for developing and implementing individualized care plans that address the unique needs of each patient.
- Work closely with physicians, clinicians, and other healthcare professionals to gather information and develop treatment plans.
- Utilize evidence-based practices to provide problem-focused interventions, education, and training activities.
- Collaborate with the care team to ensure seamless coordination of care and achieve optimal patient outcomes.
To succeed in this role, you must have:
- A graduate degree from an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
- A current nursing license in the practicing state.
- A valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
