We are seeking a skilled Travel Acute Care RN Case Manager to join our team. With the right combination of skills and experience, this individual will provide exceptional care to patients and navigate complex healthcare systems with ease.
Job SummaryThe Travel Acute Care RN Case Manager will be responsible for coordinating patient care across multiple settings, including hospitals, clinics, and rehabilitation centers. This role requires strong communication and problem-solving skills, as well as the ability to work effectively in fast-paced environments.
Key Responsibilities- Assess patient needs and develop comprehensive care plans that address medical, emotional, and social requirements.
- Coordinate with healthcare teams to ensure seamless transitions between care settings and minimize delays or disruptions.
- Monitor patient progress, identify potential issues, and implement interventions to prevent complications or readmissions.
- Maintain accurate records and documentation, adhering to regulatory requirements and organizational standards.
To be successful in this role, the ideal candidate will possess:
- A valid nursing license and relevant certifications (e.g., CCM, ACM).
- Experience working in acute care settings, preferably with travel nursing assignments.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively within interdisciplinary teams.
- Strong analytical and critical thinking skills, with attention to detail and accuracy.
