Home Health Physical Therapist Position

About the Job:

We are seeking a skilled Home Health Physical Therapist to join our team. The ideal candidate will have experience working in a hospital setting and be able to provide top-notch care to patients in their homes.

Key Responsibilities:

  • Provide physical therapy services to patients in their homes
  • Develop and implement personalized treatment plans
  • Collaborate with healthcare teams to ensure seamless care

Requirements:

  • Bachelor's degree in Physical Therapy
  • State licensure and certification as a Physical Therapist
  • At least 1 year of paid experience in a hospital setting within the last 3 years
