,margin-bottom:0'>Process referrals for continuing care services, including durable medical equipment, patient transfer, or extended care facility beds.Support the coordination of the patient intake process, reviewing, screening, verifying, and recording information from admission to completion of care.Coordinate case management activities within the assigned unit/team and serve as liaison with patients, internal teams, and external departments.Communicate with patients and/or family members to assist with requests for information, forms, and other related data.

Requirements:

A minimum of one year's experience working in healthcare, customer service, language services, or hospitality is required. A bachelor's degree is preferred. Proficiency in a second language is highly valued, particularly Spanish, Cantonese/Mandarin, or Arabic. International life experience and experience serving an international clientele are also essential for this role.
