SalaryThe salary for this position is $49,100 - $67,500 per year, based on full-time employment (40 hours per week).
Pay may vary based on individual skills, knowledge, experience, education, and certifications.
Job DescriptionAs a member of our care team, you will work collaboratively with physicians, clinicians, and other healthcare professionals to develop and implement personalized care plans that meet the unique needs of each patient.
- Provide high-quality nursing services under the direction of a registered nurse in compliance with state regulations and industry standards.
- Develop and implement treatment plans tailored to the specific needs of each patient, incorporating problem-solving strategies, teaching, and training activities.
- Communicate effectively with the care team, patients, and families to ensure seamless coordination of care.
To succeed in this role, you must have:
- A graduate degree from an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
- A current nursing license in the practicing state.
- A valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
