Our team is seeking a skilled and compassionate Licensed Practical Nurse to join our Home Health division.

This dynamic role offers the opportunity to make a meaningful impact in the lives of patients, providing personalized care and support in the comfort of their own homes.

Salary

The salary for this position is $49,100 - $67,500 per year, based on full-time employment (40 hours per week).

Pay may vary based on individual skills, knowledge, experience, education, and certifications.

Job Description

In this role, you will work closely with a dedicated team of physicians and clinicians to develop and manage care plans that support recovery and help patients regain independence.

  • Provide professional nursing services under the direction of a RN in compliance with state licensure/certification requirements and healthcare standards.
  • Utilize a holistic approach to provide problem-specific therapeutic interventions, teaching, and training activities in accordance with the plan of care.
  • Confer with the RN Supervisor regarding needed changes in the Plan of Treatment.
Required Skills and Qualifications

To be successful in this role, you must possess:

  • A graduate degree from an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
  • A current nursing license in the practicing state.
  • A valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
